What is the Perform Assessment module in RiskWatch?

By Default we are showing active assessments in the current year.You can filter by Year wise from Select Year dropdown. Current Year Assessments can be displayed by selecting 2025 year, Previous Year Assessments can be displayed by selecting 2024 year. By selecting All Data, all the Assessments displayed. It sits inside the RiskWatch compliance workspace and shares its data with assessments, the evidence vault, and the audit-pack export so a single change updates every downstream view.

How do I use the Perform Assessment screen?

Open the RiskWatch app, navigate to the Perform Assessment module from the main menu, and apply the filters or actions described in the guide below. Permissions follow your role profile, so admins see configuration, assessors see workflow actions, and viewers see read-only data. Saved views and filters persist per user across sessions.

User Guide: Perform Assessment for Compliance Management Application

Welcome to the Perform Assessment of our Compliance Management application! This feature allows perform the Survey with different response types for Risk Assessments. Risk assessment is a process for identifying and evaluating potential hazards in a workplace and deciding how to reduce risk. This guide will walk you through the functionality and capabilities of the Perform Assessment in detail.

  1. Overview of Perform Assessment

The Risk Assessment Survey aims to collect insights from employees about their understanding of the organization’s risk-taking capacity. Your feedback will help us assess our current practices and identify areas for improvement to align risk-taking strategies with organizational goals.

The Risk Assessment Survey is designed to evaluate employees' understanding of the organization’s risk-taking capacity. This flexible questionnaire ensures a comprehensive understanding by incorporating varied response types, including yes/no, rating scales, multiple-choice, dropdowns, File upload, Custom response, text, Date and Number.

  1. Perform Assessment screen details

Perform assessments screen shows with following details

  • All the Assessment details
  • Facility which are mapped with Assessments
  • Progress of the Assessments
  • Status of the Assessments such as New, In Progress, Completed, Closed and Incomplete.
  • End Date of the Assessments
  • Questions details such as Total Questions and Total Answered.

Survey information can be uploaded directly from the sheet, Using Download data icon, sheet be will downloaded and responses will be added and same sheet will uploaded using Upload Data icon.

  1. Assessment Survey screen details

Assessment Survey screen displayed once clicked on the Assessment and following details are available.

  • By Default Survey page displayed when clicked View Details button on the Perform Assessment screen for Particular Assessment. Survey page also displayed when opened Assessment Summary section from Assessment screen.
  • Introduction tab, shows the information of Introduction, Assessment Instruction and Narrative.
  • Take Survey tab, shows the following information for the response types yes/no, rating scales, multiple-choice, dropdowns, File upload, Custom response, text, Date and Number
    • Question categories and associate questions details.
    • We can attach the file and using @ option email id we can notify someone.
    • Viewing the recommendation mapped to this question and add new recommendation.
    • Justification of Implementation information can be added
    • Review toggle option can be selected
    • Added Response can be removed from Remove Response toggle
    • Survey can also be uploaded using download data sheet and uploaded the data sheet.
  • Findings tab shows the information any findings added
  • Evidence Collections tab, we can add evidences using Add Evidence button. Bulk upload of evidences using sheet. We can clone the earlier added evidences using Clone.
  • Risk Assessments information also displayed which are selected while creating the Assessment.

To Access Perform Assessment screen, follow below steps

  • Navigate to Main Menu
  • Select Perform Assessment menu

    • Navigate to Main Menu

    • Select the View Details button for the Assessments
    • Go to Assessment Summary
    • Select Perform Assessment column
    • Click on Perform Assessment icon
    • Survey screen is displayed for that assessment
  2. Filter Options in Perform Survey Screen

Take Survey screen we have following filter options

  • All – All the Questions in the Survey are displayed
  • Answered – Only answered questions are displayed
  • Unanswered – Only Unanswered questions are displayed
  • Review – Only those questions displayed, which are toggled as Review
  • Comments – Only those questions displayed, which are having comments added
  1. Language Options for Perform Assessment

To cater to diverse user needs, the Perform Assessment screen supports multiple languages. You can select your preferred language from the dropdown menu located at the top of the screen. This ensures that Perform Assessment screen displayed in the selected language, accessible and understandable across global teams.

  1. Perform Assessment Events

Events shows the information of all the logs of the Perform Assessment updated. When response are updated events action performed displayed as ‘Updated’.

Following functionality available in Perform Assessment events screen

  • Filter options available and we can run and save the filters. Saved filters are reused for future use.
  • Column Sorting is available for Sort By option by selecting the columns names Action Performed, Performed By, Module, Screen, Action Date and Action Info and View details
  • Search option available by searching any word or text, if search text is available, it will display the search content.
  • Downloading data of Survey logs displayed in pdf format.
  • Column display options available using hide/seek columns, if columns are not selected columns will not displayed in tasks events.

  • Select the ‘Facility Name’ in the Sort By field, Facility Name column is sorted.
  • Select the ‘Progress’ in the Sort By field, Status column is sorted.
  • Select the ‘End Date’ in the Sort By field, End Date name column is sorted.
  • Select the ‘Total Questions’ in the Sort By field, Total Questions column is sorted.

  • Open the Performance screen

  • Click on See/hide column
  • Selected list items columns are displayed on the screen
  • Un selected columns are not displayed on the screen

  • Go to Search field
  • Enter the data in the search field
  • If searched record available on the screen, it shows on the screen
  1. How to upload responses using sheet?
    • Navigate to Main Menu
    • Select Perform Assessment menu
    • Click on the View details icon for the required Assessment Survey
    • It displayed Perform Survey screen
    • Click on Download icon at right upper side of the screen
    • Enter the Response for the questions such as yes/no, Scale, Custom Responses and text and Number
    • Upload the sheet
    • Uploaded Responses added to the Survey

    • Navigate to Main Menu

    • On the Add Finding pop up screen
    • Enter the details Finding Text, Finding weight, Task Priority

    • Navigate to Main Menu
    • Select Perform Assessment menu
    • Click on the View details icon for the required Assessment Survey
    • It displayed Perform Survey screen
    • Go to Evidence Collections tab
    • If Any Evidence added, those are displayed
    • To add new collections, click on Add Evidence button
    • On the Evidence Information pop up, enter Evidence and description
    • Click on Save
    • Added Evidence collection displayed in the screen
  1. How to add evidence collections in bulk using sheet to the Assessment?
    • Navigate to Main Menu
    • Select Perform Assessment menu
    • Click on the View details icon for the required Assessment Survey
    • It displayed Perform Survey screen
    • Go to Evidence Collections tab
    • If Any Evidence added, those are displayed
    • Click on Download to download the sheet
    • Enter the evidence collections and upload
    • All the uploaded Evidence Collections are displayed on the Screen
  1. How to copy or clone the Evidence collections from other assessments?
    • Navigate to Main Menu
    • Select Perform Assessment menu
    • Click on the View details icon for the required Assessment Survey
    • It displayed Perform Survey screen
    • Go to Evidence Collections tab
    • If Any Evidence added, those are displayed
    • Click on Clone
    • It displayed the pop up screen, to select the Assessment and Facility
    • Click on Clone
    • All the Evidence collections from other Facilities are copied and displayed on the screen
    • Duplicate clone displayed will not cloned
  1. How to edit or delete the Evidence collections?
    • Navigate to Main Menu
    • Select Perform Assessment menu
    • Click on the View details icon for the required Assessment Survey
    • It displayed Perform Survey screen
    • Go to Evidence Collections tab
    • Using edit icon, added evidence is edited
    • Using delete icon, added evidence collection is deleted
  1. How to save the response, when it is showing ‘Comment is mandatory’?
    • Navigate to Main Menu
    • Select Perform Assessment menu
    • Click on the View details icon for the required Assessment Survey
    • It displayed Perform Survey screen
    • Enter the comments fields, when it is showing mandatory
    • Question response will be saved.
